Donated Dental Services (DDS) is a Wisconsin Dental Association program. The program is directed to people who are unable to afford needed dental care because of a limited income which is clearly linked to a permanent disability, chronic illness or advanced age (65 and over).

Marquette University School of Dentistry Dental Clinic provides dental care for about half the price of a private dentist. Participating clinics are located both in and outside of Milwaukee. The Marshfield Clinic Park Falls Dental Center opened March 3, 2008. The dental center is open 8 a.m.-5 p.m., Monday – Friday. A sliding fee scale will assist those with limited income and no insurance. You can schedule an appointment by calling 715-762-0200 or by visiting the Center's web site.
The Marshfield Clinic Chippewa Dental Center serves all patients regardless of the insurance or ability to pay. The Northern Virginia Dental Clinic is a private non-profit 501(c) (3) organization. It was established in 1994 by members of the Northern Virginia Dental Society who wanted to contribute a critically-needed service to the community while preserving the dignity of the underserved. A unique, public/private partnership between dental professionals, local governments and social service agencies was formed.
Eligibility for Care
The NVDC accepts only those individuals deemed eligible through an established network of referring social service agencies.
Individual must be a resident of:

- Fairfax County
- Loudoun County
- City of Alexandria
- City of Fairfax
- City of Falls Church
Additionally, a small percentage of Prince William County residents may be eligible for care. A higher fee for service applies.
Individuals must be age 18 or older.
Individual/family income must be at or below 200% of the federal poverty guidelines.
While eligibility is determined by the referring agencies, the NVDS reserves the right to request a reassessment of eligibility or to refuse treatment.

Fees for Service
Most services are offered at a flat rate of $40 per visit. Additional fees are charged for biopsies and prosthetic appliances.
All fees must be prepaid. No appointment will be scheduled prior to receipt of payment in full.
Payments may be made in the form of cash or money orders. No personal checks will be accepted.
Appointments
Emergency and initial appointments are scheduled through designated referring agencies only. The initial appointment will require approximately one hour so that a medical history can be obtained, a thorough examination with x-rays will be conducted, and a detailed treatment plan will be developed. Referring agencies will be advised of appointment availability.
Individuals requiring additional treatment may schedule appointments prior to departing the clinic. Appointments may be rescheduled. However, rescheduling must occur at least two working days prior to the originally scheduled appointment date/time, or appointment and payment will be forfeited.
No refund will be made for missed appointments or to individuals arriving more that ten minutes late for their scheduled appointment.

Clinical Services
Our clinical office, located on the second floor of the main Health Department, provides emergency care, routine dental services to qualified (see below) Knox County residents. The dental clinic is open from 8:00 a.m. until 4:30 p.m. Monday through Friday.
Qualifications:
Knox County TennCare Dental Insurance recipients (age up to 21st birthday) may call the Dental Clinic at 215-5110 directly to schedule appointments.
All others (adult and children) must be qualified through the Health Department's Social Services Department at 215-5400 before scheduling an appointment in the dental clinic. Proof of income and Knox County residence is required.
Emergency Dental Care
Emergency care is provided to residents of all ages residing in Knox County and who meet certain income guidelines. Emergency care for relief of pain generally consists of extractions. Patients are offered the opportunity to seek services from the private dental community if less invasive procedures are an option. There is a $5.00 administrative fee for each visit.
Routine Dental Services
Routine dental services are provided to Knox County residents until their 21 st birthday. Services are provided on a sliding fee scale based on family income. Fees may slide to zero pay for qualifying low income. TennCare dental insurance is accepted with no charge to the patient/parent.
Routine services include dental cleanings, x-rays, oral hygiene instruction, fluoride treatments, sealants and fillings for both the primary and permanent dentition. Other dental procedures offered on a limited basis include root canal treatment on adult anterior teeth, space maintainers and stainless steel crowns.
School-Based Services
School-based services program provide preventive dental sealants, oral hygiene education, dental screening and referral to private dentists as appropriate. These services are offered free of charge to all children in grades K– 8 at selected schools. Schools are selected based on the percentage of students at the school receiving free and reduced lunches. Schools with greater than 50 percent (50%) of students receiving free lunches can be selected for the program.

Hundreds of disabled and elderly citizens throughout Montana have seriously-neglected dental problems. Because of their disabilities or ages, many cannot work.. Montana’s Medicaid program provides only limited dental services and Medicare generally does not provide dental treatment. Some people do not qualify for assistance even if they are disabled. As a consequence many disabled and elderly people suffer because they cannot pay for the dental care they need.
The Montana Donated Dental Services (DDS) program was established in 1997 to help some of our most disadvantaged citizens: people who are disabled, elderly, or medically compromised. The dentists volunteering for DDS donate services in their own offices. Dental laboratories also contribute services. Since the DDS program was created, Montana dentists and dental labs have donated more that $1 million of free, comprehensive dental treatment to those who cannot afford treatment due to financial limitations associated with their age or disabilities.
Currently, nearly 150 dentists and more than 65 dental laboratories across Montana serve as volunteers for DDS. The Montana Dental Association donates office space for the program coordinator.
Patient Qualifications
To qualify for the DDS program, a patient must:
- Have a permanent disability or be elderly (over age 62). The disability may be physical or psychological in nature. It must be a permanent condition that prohibits or significantly limits employment.
- Be within 150% of the federal poverty guidelines and unable to pay for dental treatment. They must also show that they have no other means of obtaining needed dental care.
- Need extensive (comprehensive) dental treatment, i.e. more than a check-up and cleaning. The process for application and treatment under the DDS program is as follows:
Application Process
A prospective patient submits an application for donated dental care that is screened by telephone during an intake interview with the program coordinator, Margaret Petrick, who determines eligibility for the program. She contacts the appropriate social service agency or medical professional for further information if eligibility is questionable.
Matching Process
The applicant is then matched with a volunteer dentist when one becomes available in the applicant's area. The dentist receives a patient profile outlining the applicant's dental needs, health issues, and financial circumstances prior to accepting and treating the patient.
Treatment Process
The applicant receives a letter reporting the phone number and address of the volunteer dentist. The patient schedules an initial appointment with the dentist. After the initial examination, the program coordinator contacts the dentist and confirms that the patient was on time for the appointment and the dentist agrees to continue to see the patient. The program coordinator arranges for lab work and services by a specialist. During the span of the treatment, the dentist or patient may contact the coordinator at any time if either experiences any problems with the program.
Completion Process
After treatment is complete, the dentist reports the time involved in the case and value of the services provided. After completion of the DDS process, the patient is ineligible for future services, so the DDS program may treat other eligible persons.
DDS is always in need of dentists and labs willing to assist. Volunteers may elect to see one or more patients per year, and treatment is always at the dentist’s discretion.

Dental Health Arlington (DHA) is a nonprofit organization, which opened its doors in 1993 to provide access to care for low-income residents of southeast Tarrant County, Texas. Our mission is to provide preventive and pain relieving services to low income residents as well as to educate children about good oral hygiene habits. Each year DHA provides free and low-cost dental care for approximately 2,500 people of all ages in our clinic. SMILES school-based prevention program helps 8,000 disadvantaged elementary school children in Arlington, Mansfield and Fort Worth who might otherwise have gone without care.
